Who can be Referred to a Dietitian

Patients who have or need advice on: -

  • Diabetes
  • Patients with Diabetes who develop additional problems e.g.
    • Micro or Macro Albuminuria
    • Hypercholesterolaemia
    • Poor Glycaemic Control
    • Obesity
    • Hypertension Impaired
  • Glucose Tolerance
  • Swallowing problems and modified consistency diets
  • Malnutrition/feeding Difficulties
  • Nutrition Support
  • Nutrition/food related problems due to mental health state or medication
  • Nutritional Advice
  • Allergy/food Intolerance/Coeliac Disease
  • Eating Disorders (Compulsive/Binge Eating Disorder/Bizarre/ Restricted Eating Patterns)
  • Children requiring any modified diet
  • Obesity

Helpful Information to Include in the Referral

As much information as you can give will help us to priortise and give your patients the best service. Please include if appropriate:-

  • Relevant social information
  • Previous relevant medical, weight history
  • Relevant medication prescribed
  • If language interpretation is required
  • Recent relevant biochemistry
  • If ambulance transport is required
