There are facilities available for the patient to be seen as an out patient in the acute hospital, in satellite units and in the patient's own home.

Referral Criteria

Any patient aged 16 or over with a neurological disorder, requiring rehabilitation due to a change in their condition, can be referred.

Referral Pathway

Referrals from fellow therapists on discharge from hospital, Consultants, GPs and other members of the MDT in the hospital and in the community can be accepted.

Additional Information

Patients can be seen individually or in groups. Upon receiving referral, patients will be categorised as to the priority of need for treatment ie. hospital discharges will be a high priority along with urgent community referrals.
