Occupational Therapy team based in Adult Therapy Services, BRI ext 4306,

Deliver service to wards 3, 6 and 9

Referral Criteria

Patients admitted with acute or chronic conditions leading to functional deterioration or mobility problems

All patients admitted with stroke or recurrent falls

Referral Pathway

Informal referral via MDT on a day to day basis.

Weekly M.D.T. meetings on ward 9 only
