Location

Horton Park Centre or Home based therapy covering patients within the former city PCT area.

Referral Criteria

Home environment needs therapy input to assess equipment provision.

To accommodate the patient where travelling to a centre would be psychologically or physically inappropriate.

To accommodate the patient's physical health in relation to risk associated with physical problems and manual handling.

To accommodate risk to staff with manual handling.

Treatment will take place within the centre when the patient would benefit psychologically from being outside the home environment and where more specialist equipment/supervision is required.

Referral Pathway

Referrals accepted from GP, Physiotherapist, Occupational Therapist, District Nurse, Social Services, FRNS, Rapid Access Clinic, MDT (DH), Self Referral, NH/RH (GP first). Allocated and prioritised Urgent 2 working days, Other 10 working days. If Referred on to groups etc., Referrer is informed and a letter sent.

Additional Information

To provide rehabilitation to predominantly elderly adults with acquired/progressive/long standing physical disability problems, within their home or with centre based treatment.
