Mainly over 65

Needing assessment / treatment in home situation

Unable to travel e.g. frailty, where trauma of travelling outweighs the benefits of treatment

Where the patient is also a carer

CRISIS INTERVENTION TEAM

Referral Pathway

Written referrral from: GP, District Nurse, Social Services, Physiotherapy, Occupational Therapy, Consultant, Rapid Access Clinic, Crisis intervention team.
