Referral Criteria

Patients with a known diagnosis of along term condition requiring a M. D. T. assessment involving a full medical screen.

All patients are assessed by Dr Chu, Angela Ashcroft and a senior therapist who decides if the patient would benefit from therapy intervention.

Referral Pathway

GP, Rapid Access Clinic, Consultants, Falls Nurse, DN, Physiotherapist, Occupational Therapist, Fast Response Team

Additional Information

Many of the patients have multiple problems so the team ensures patients are treated holistically, considering their medical needs, therapy needs, cognitive abilities and home circumstances.
