Referral Criteria

There are two beds in Meadowcroft available for clients from City Social Services or City PCT. Patients should be over 65 and referred from either Social Services or the Nurse Case Manager. After discussing the option with the user and the carer, then either the Meadowcroft Rehab Team or the Nurse Case Manager and the GP should be approached. If all are in agreement, the patient should then be screened for suitability before a bed is booked.

Referral Pathway

Referrals accepted from GP, Physiotherapist, Occupational Therapist, District Nurse, Social Services, FRNS, Rapid Access Clinic, MDT (DH), Self Referral, NH/RH (GP first)

To provide rehabilitation to elderly adults with acquired/progressive/long standing physical disability problems, within their home.
