Referral Pathway

The team accept referrals from GP's, School Medical Officers, Consultants or Registrars.
Referrals go to a multidisciplinary allocation meeting.
Children are then usually seen by the Physiotherapist who works in the particular postcode area of the child's home address or school that they attend.
Different therapists also tend to specialise in working with different ages of children, or with special types of medical problems.

The Child will have an assessment at the therapy base in the Child Development Centre, school, or occasionally at home with parent / carers present.
Input from the service will be in accordance with individuals needs, and may include:
Therapy advice/programme
Individual therapy and / or referral to a group session
Referral to other agencies
Discharge or long term monitoring with self referral form
Training and liaison with staff of other agencies (e.g. education), to support the implementation of a therapy programme
