Children in the home setting and community requiring Physiotherapy support.

Referral Pathway

Referrals for the Children's community Team at Horton Park from Consultants / GP's / School Nurses / Community Nurses / Physiotherapist / Education & Social Welfare / Families.

Additional Information

Provide support for children / families / carers and professionals caring for children with life limited/ threatening conditions. Provide Bereavement support after death to families / carers.
Provide training on caring for children with life limited conditions and bereavement care.
