Acute Care For Older People - In Patients BRI

Therapy team based in Adult Therapy Services ext 3161

Deliver service to wards 3, 6 and 9

Referral Criteria

Patients admitted with acute or chronic conditions leading to functional deterioration or mobility problems

All patients admitted with stroke or recurrent falls

Patients with severe disability may be referred for advice re positioning, seating,  manual handling

Respiratory conditions appropriate for physiotherapy will be seen by specialist respiratory team

Referral Pathway

Informal referral via medical or nursing staff on a day to day basis.

Weekly M.D.T. meetings on ward 9 only
