Referral Criteria
Rehabilitation Unit: Usually 65+ but can be younger.
To avoid hospital admissions patients requiring intensive therapy and 24hr care. Patients requiring further rehab after hospital stay.
Patients needing to improve their functional skills.
Respite: Staff require advice on patient's mobility, transfers etc. Patient requires treatment/advice. Pain, reduced mobility, reduced joint movement, weakness.
Residential: Patients who have been referred for physiotherapy.
Referral Pathway
Rehabilitation Unit: Care Manager, Case Manager, BRI Physiotherapy, SLH Physiotherapy, Occupational Therapy, GP, District Nurse, Fast Response Team Respite: Staff, Care Manager, GP
Residential: Staff, GP
Additional Information
Most patients are assessed by a therapist and the Neville Grange Manager/Care Manager before being admitted to the rehabilitation unit
