Referral Criteria

Mainly over 65

16+ if completely housebound

Needing assessment / Treatment in home situation

Unable to travel e.g. frailty, where trauma of travelling outweighs the benefits of treatment

Where the patient is also a carer

Chest infection Problems

Crisis intervention team referrals

Referral Pathway

Written referral from: GP, District Nurse, Social Services, Physiotherapy, Occupational Therapy, Consultant, Crisis intervention team, Elderly asssessment service.

Additional Information

To provide rehabilitation to predominantly elderly adults with acquired/progressive/long standing physical disability problems, within their home or with locality centre based treatment.
