A specialist service as part of the Community Palliative Care Team for patients and relatives in community settings. Working with multidisciplinary team to offer holistic care for peiple with both cancer and non cancer diagnoses.

Locations

At Marie Curie Hospice both as inpatients and outpatients, in their own homes, nursing homes

Referral Criteria

Psychological problems related to Palliative Care e.g., low mood, depression, adjustment to illness, grief, loss, bereavement issues. MDT and staff support.  We will see relatives of patients

Referral Pathway

Referrals from Gp's, consultants, Palliative Care Team

Additional Information
